How To File A Claim

1. If equipment is lost or stolen, customer must report the incident with the police department to obtain a case number or police report. This number willbe needed to process the claim.
2. In the event a claim must be filed, the customer must contact Simmetry Wireless Customer Care at 1-866-220-6867.
3. Once the claim has been filed, the customer service representative will supply you with a claim number.
4. Once you have received a claim number, the customer service representative will direct you to the nearest location to complete transaction and receive replacement handset.

Conditions

• To fulfill the claim, the primary account holder or authorized user mustbe present.
• Initial claim can not be filed within 30 days of adding Sim-Protection.
• Report and complete the claim within 30 days of the incident.
• Account must be in current status for the claim to be honored.
• Deductibles must be paid at the time your claim is fulfilled. Sim cards are not covered by Sim-Protection.
• Claims may be fulfilled with new or refurbished equipment.
• If the exact model is no longer available, we reserve the right to replace your equipment with a model having similar features and quality.
• The policy allows two claims with a 12 month period, beginning with th edate of the first incident. After the second claim is filed ina 12 month period, the customer will be cancelled from the program and may re-enroll 12 months from the date of the second replacement.
• Sim-Protection must be added at the time of activaiton. Sim-Protection can not be added once the handset has left the location of purchase.
• If a customer's account should ever be disconnected for any reason, the account would not be eligible for Sim-Protection until they purchased a new phone.
